Please bring a valid photo ID, your dental insurance card (if applicable), and any recent dental X-rays taken within the last 6 months. If you have a list of current medications, please bring that as well.
We kindly ask that you provide at least 24 to 48 hours' notice if you need to reschedule or cancel your appointment. This allows us to offer the time slot to another patient in need of care.
Our clinic is located in the heart of Forest Hills, Queens, NY. There is street parking available around the office, as well as local public parking garages within walking distance. We are also easily accessible via public transportation.
